This ratio compares 2 hydroxy estrone, a weaker and less stimulatory metabolite, to 16 hydroxy estrone, a strongly tissue activating metabolite. A higher ratio suggests estrogen is being preferentially metabolized through pathways associated with lower proliferative risk. A lower ratio indicates a shift toward prolonged estrogen receptor activation. This balance is influenced by insulin sensitivity, inflammation, gut health, and dietary patterns and is commonly used to assess long term estrogen related risk rather than short term hormone symptoms.
