Does a Zero Calcium Score Erase High LDL Risk?
A coronary calcium score of zero is one of the most reassuring results in cardiovascular prevention. It means no detectable calcified plaque in your heart's arteries right now, and it reliably marks low risk over the next several years. But if you're in your 40s or 50s and your LDL has run high for years, that zero is a snapshot, not a reset. Calcium is a late feature of plaque that starts forming years before it hardens, so a clean scan can miss disease that hasn't calcified yet. The question worth answering is when the zero should settle your prevention plan and when it shouldn't.
A zero calcium score is good news about the next several years, not proof your arteries are clean. In MESA, adults with a zero score had 10-year rates of heart attack or stroke between about 1.3% and 5.6%. In the Coronary Artery Calcium Consortium, cardiovascular death among people with a zero score stayed under 1 in 2,000 per year over a mean 12-year follow-up. Of all the negative risk markers MESA tested, a zero score shifted risk down the most. Low is not zero, and the reason matters most in younger adults with high LDL.
Calcium is the scar, not the whole disease
Atherosclerosis starts when apoB-containing lipoproteins, LDL among them, lodge in the artery wall and set off years of inflammation. Calcium shows up late, as plaque matures and hardens. A CT scan measures the calcified part of the burden. The softer, noncalcified plaque that comes first can be there while the score still reads zero.
This isn't theoretical. In a review of about 38,000 asymptomatic adults with a zero score, roughly 1 in 10 had noncalcified plaque on CT angiography, and about 1 in 100 had obstructive disease. In a cohort screened because of a first-degree family history of premature coronary disease, a positive calcium score identified only about half of plaque under age 40, rising past 90% by 65. In a small study of asymptomatic adults with type 2 diabetes and a zero score, nearly a quarter had high-risk plaque. And calcium tends to appear later in women than in men, so the same zero can mean different things at different ages.
Does LDL still count when the scan is clean?
The data are mixed. In the Western Denmark Heart Registry, over about four years, LDL was not meaningfully tied to events among people with a zero score, even at very high LDL. That's the strongest case for the scan reassuring you over a short window. A later analysis of about 24,000 symptomatic people with a zero score and longer follow-up pointed the other way: higher LDL tracked with more noncalcified plaque and higher coronary heart disease risk, most of all in those 45 and under. This isn't a clean reversal. The two studies used different endpoints, different LDL comparisons, and overlapping populations, and even in the later one only about 1 in 100 had a coronary event over seven years, a higher relative risk on a very low base.
That contrast is the point. A zero score is good at near-term risk, not at erasing lifetime LDL exposure. Cumulative LDL exposure in young adulthood and middle age was associated with later coronary heart disease independent of midlife LDL, and cumulative apoB exposure in young adulthood was associated with later ASCVD. The broader LDL evidence is dose- and duration-dependent, like pack-years for a smoker. A clean scan today doesn't subtract the years your artery wall has already spent under pressure.
When the zero should change your plan, and when it shouldn't
The 2026 ACC/AHA dyslipidemia guideline lets a zero score support deferring lipid-lowering therapy only in a selected low- or intermediate-risk person who prefers to avoid medication and has no higher-risk condition. It doesn't buy deferral if your LDL-C is at or above 190 mg/dL or you have familial hypercholesterolemia, diabetes, current smoking, or a strong family history of premature ASCVD. In familial hypercholesterolemia, calcium scoring isn't used to defer therapy at all. Even among people with LDL-C at or above 190 mg/dL and a zero score, MESA found far lower event rates than in those with calcium, though that group was older, not young adults. Treatment remains generally recommended because cumulative LDL exposure can drive disease the scan may not yet see.
The same guideline pushes treatment earlier to cut prolonged exposure to atherogenic lipoproteins, and considers medication for young adults with LDL-C at or above 160 mg/dL or a strong premature family history. There's no LDL-C level below which the benefit of lower lifelong exposure clearly stops. So the move after a zero score isn't to close the book. Check your LDL-C and apoB, look at how long they've been elevated, and weigh the scan against family history, smoking, diabetes, and your 10- and 30-year risk. If therapy is deferred because of a zero score, the guideline recommends reassessing with repeat CAC testing in 3 to 7 years depending on baseline risk.
What would change this judgment is randomized evidence showing that using a zero score to defer LDL-lowering is as safe over the long run as treating earlier in younger adults with sustained high LDL. That evidence is not yet available. A zero calcium score buys reassurance about the near term, not immunity. In a younger adult with years of high LDL, the clean scan should inform the prevention plan, not retire it.
