About the medication

A simple daily routine, backed by data.

One small pill, once a day. Most patients see measurable LDL reduction within a few days, with steady-state effect by about week two.

  • Take once dailyAny time of day, with or without food. Pick a time that fits your routine and stay consistent.
  • No titration requiredOne 10mg dose works for everyone. No dose escalation, no timing tricks.
  • Works on its own or with a statinEzetimibe lowers LDL by about 15 to 20% on its own. Added on top of a statin, it drops LDL another 25%. The 2018 ACC/AHA guidelines name it as the first-line non-statin add-on when a statin alone isn't enough.
  • Recheck lipids at 4 to 12 weeksWe recommend a follow-up lab at 4 to 12 weeks to confirm your response, even though most of the LDL drop shows up within the first two weeks.
Why ezetimibe

The first non-statin add-on proven to cut cardiovascular events.

Ezetimibe selectively blocks the NPC1L1 transporter in the small intestine, the protein that pulls dietary and biliary cholesterol into the body. Blocking it means less cholesterol reaches the liver, so the liver pulls more LDL out of the bloodstream. It's mechanistically distinct from statins, which is why it can meaningfully lower LDL on its own and why combining the two works even better.

  • Works on its ownAs monotherapy, ezetimibe lowers LDL by roughly 15 to 20%, a meaningful drop without a statin. It's a real option for people who can't tolerate statins or prefer to start with a non-statin.
  • Even stronger with a statinAdded on top of a statin, ezetimibe cuts LDL by another 25%. IMPROVE-IT proved that combination further reduces heart attacks and strokes in high-risk patients, validating the "lower LDL is better" principle.
  • A different target, gentler profileEzetimibe works in the gut, not the liver, and side effects as monotherapy are comparable to placebo. No titration, no timing tricks, one 10mg tablet a day.

The IMPROVE-IT trial

Published in NEJM in 2015, IMPROVE-IT followed 18,144 post-heart-attack patients for six years. Half took a statin alone; half took the same statin plus ezetimibe. The combination group had lower LDL and fewer cardiovascular events. It was the first trial to prove a non-statin add-on cuts event risk, validating the principle that lower LDL is better.
