Ezetimibe selectively blocks the NPC1L1 transporter in the small intestine, the protein that pulls dietary and biliary cholesterol into the body. Blocking it means less cholesterol reaches the liver, so the liver pulls more LDL out of the bloodstream. It's mechanistically distinct from statins, which is why it can meaningfully lower LDL on its own and why combining the two works even better.
Published in NEJM in 2015, IMPROVE-IT followed 18,144 post-heart-attack patients for six years. Half took a statin alone; half took the same statin plus ezetimibe. The combination group had lower LDL and fewer cardiovascular events. It was the first trial to prove a non-statin add-on cuts event risk, validating the principle that lower LDL is better.
