What Helps Prevent and Treat Wrinkles?
The best-proven way to prevent wrinkles is also the least glamorous: daily broad-spectrum sunscreen. In a randomized trial, steady daily use showed no detectable increase in skin aging after 4.5 years. Once fine lines start, topical retinoids have the strongest treatment evidence, and prescription tretinoin is the benchmark. Botulinum toxin, fillers, and resurfacing can each soften a specific kind of existing line, but they are not broad prevention tools. The plan that holds up is layered, not one product.
Much visible facial aging is sun damage. Smoking and air pollution add to extrinsic aging, but UV is the driver you can most directly block.
Daily sunscreen has the strongest prevention data
The anchor is that trial: 903 Australian adults younger than 55, followed 4.5 years. Daily broad-spectrum SPF 15 or higher meant about a quarter less skin aging than using it whenever people felt like it, measured on the back of the hands. That's the UV-driven process being interrupted, not a surface cosmetic effect. The product was a 1990s SPF 16 with weaker UVA filtering than many current broad-spectrum options, so the trial probably did not test the strongest modern UVA protection.
A one-year, open-label trial across skin phototypes II through VI adds support: high-protection sunscreen cut the progression of several wrinkle and dark-spot measures by a third to a half. The biology fits. UVA reaches into the dermis and generates reactive oxygen species that break down collagen and elastin, so daily broad-spectrum coverage is what stops the damage. Guidance points to broad-spectrum SPF 30 or higher, applied generously and reapplied about every two hours outdoors.
The catch is dose. SPF 30 blocks about 97% of UVB in testing, but thin or skipped application quietly erases that. If you're prone to dark spots, a tinted sunscreen with iron oxides also blocks visible light that plain UV filters miss, though that benefit is documented for pigmentation, not wrinkles.
Retinoids repair, but on a timescale of months
Once fine lines exist, prescription tretinoin has the best evidence for improving them. Across randomized vehicle-controlled trials and reviews, it improves fine wrinkling most clearly, with measurable gains within a few months and most of the benefit over roughly 6 to 12 months. Continued use is what maintains it. The mechanism is partial repair: in a landmark study, 0.1% tretinoin over 10 to 12 months raised a measure of collagen I formation by about 80% in photodamaged forearm skin, while vehicle-treated skin lost collagen.
Tretinoin's weakness is tolerability. Redness, peeling, and burning are common, worst in the first weeks, and they're the main reason people quit. That's where retinol or retinaldehyde earn their place. Retinol is weaker, but a 26-week vehicle-controlled trial found it improved fine and deep wrinkling with relatively low irritation. Retinaldehyde has smaller, mixed evidence and generally irritates less than retinoic acid. The gentler retinoid you'll actually use beats the strong one you abandon.
Two limits matter. A 2025 network meta-analysis found tretinoin clearly effective for fine wrinkles but not for coarse, deep ones; among retinoids, tazarotene did the most for coarse wrinkles, though it irritates more. As a class, retinoids do more for texture and fine lines than for deep folds, and tretinoin softens fine wrinkles rather than erasing them or reversing sun damage. Tretinoin can also make skin more sun-sensitive, so it works alongside daily sunscreen, not instead of it. It's not for use in pregnancy.
Procedures soften specific lines, not the underlying process
Injectables and resurfacing work by different mechanisms, which is why matching the tool to the line matters more than picking a favorite. Botulinum toxin relaxes the muscles behind dynamic expression lines. Fillers add volume under static folds. Lasers, peels, and microneedling work on texture, photodamage, and collagen remodeling. Most studies test one modality at a time rather than head-to-head. A small randomized trial shows why combination can help when targets differ: in 40 people with crow's-feet, botulinum toxin plus a collagen filler reached a markedly effective result in 90%, versus 40% for toxin alone, judged on a subjective scale over six months.

The safety picture is reassuring but not trivial. Botulinum toxin's adverse events are usually mild: headache, a drooping eyelid or brow, swelling, injection-site soreness. The label also warns about rare distant spread of the toxin. Care is more individualized with neuromuscular disorders, pregnancy or lactation, active infection at the site, or interacting medicines.
Fillers are mostly bruising and swelling. The serious risk is accidental injection into a blood vessel, which can cause skin death, stroke, or blindness. If you're the one with a severe event, none of that feels minor. That's the case for a licensed, trained injector instead of bargain shopping.
Where the evidence is thin
Some popular options do less than the marketing implies. Oral collagen supplements show pooled wrinkle signals in some meta-analyses. Oral astaxanthin improved skin moisture and elasticity but didn't reduce wrinkle depth in pooled human trials. Many over-the-counter retinoid derivatives, especially retinyl esters like retinyl palmitate, mostly lack properly designed trials. Facial exercises have no comparable controlled evidence for wrinkle prevention. Plain moisturizer can make lines less noticeable by hydrating skin, but that isn't retinoid-level treatment. None of this proves they do nothing. They just don't belong ahead of sunscreen and a retinoid.
The plan is simple but not instant: protect skin from UV every day, add a retinoid if you want proven gradual repair and can tolerate it, and save procedures for the specific lines skincare can't reasonably change. What would sharpen the order is a large, long-term trial comparing a sunscreen-plus-retinoid routine head to head against procedures, supplements, and drugstore regimens on wrinkles people can actually see. Until then, the order of operations is the answer, and it favors prevention first.
