This test is most useful if any of these apply to you.
A normal ALT is one of the most reassuring numbers on a standard blood panel, and one of the easiest to overread. Many people with fatty liver or even fibrosis have an ALT that stays inside the lab's normal range.
That does not make the test useless. It makes it a starting point. When ALT is up, it usually means liver cells are spilling their contents into your blood. In a healthy-looking adult, a common reason is fat building up in the liver.
ALT (alanine aminotransferase) is an enzyme. Enzymes are proteins your cells use to speed up chemical reactions. ALT helps the body handle amino acids and make glucose. It lives mostly inside the fluid of liver cells, with smaller amounts in muscle and a few other tissues.
Because it is packed into liver cells, ALT stays low in the blood until those cells are damaged. When a cell's outer membrane is breached or the cell dies, the enzyme escapes and the blood level climbs. So ALT is an injury signal, not a report card on how well your liver is doing its everyday work. That is why doctors increasingly call it a liver chemistry rather than a liver function test.
ALT is liver-enriched, but not liver-only. A recent hard workout can nudge it up through muscle, which matters when you read a single result.
The most common cause of a mildly raised ALT in an otherwise healthy adult is fat accumulating in the liver, now usually called metabolic dysfunction-associated steatotic liver disease, or MASLD. Older papers call it NAFLD. Higher ALT is more common in the inflamed, more damaging form, MASH, but it is still a poor gauge of how far the damage has gone.
In a study of adults with type 2 diabetes screened for liver disease, only about 13 in 100 with fatty liver had ALT high by a conventional lab standard. Among those with fibrosis, only about 28 in 100 had a high ALT. The number can be reassuringly normal while the tissue underneath is not.
So a normal ALT is worth having, but if you carry the metabolic risk factors that drive fatty liver, it is not a reason to stop looking.
A raised ALT can be one of the blood clues that the liver is becoming resistant to insulin before diabetes is diagnosed. In the Yuport Japanese cohort, people in the highest ALT group had roughly 80% higher risk of developing type 2 diabetes than those in the lowest group, after accounting for weight, blood pressure, and lipids.
A creeping ALT alongside a widening waistline is a metabolic warning, not just a liver footnote. Check fasting glucose, HbA1c, and insulin sooner rather than later.
Fatty liver is common, but a high ALT has a wide differential. Viral hepatitis B or C, heavy alcohol use, acetaminophen and other drugs, inherited iron overload, autoimmune hepatitis, and Wilson disease all injure liver cells and can push ALT up.
This is why an unexplained elevation is not something to shrug off or to blame on one guess. The workup exists because several of these causes are treatable and quiet in their early stages.
ALT is not a simple lower-is-better marker. Across large populations, the relationship between ALT and death rates looks like a valley: risk rises at both very low and fairly high levels, with the lowest risk somewhere in the middle.
Very low ALT tends to travel with low muscle mass, frailty, and reduced metabolic reserve. In the ARIC cohort, adults in the bottom tenth of ALT had about a quarter higher later dementia risk after full adjustment. In a cohort of people with chronic lymphocytic leukemia, the low-ALT group had about 30% higher adjusted risk of dying.
Both ends can be true at once. ALT is a readout of two different things pulling in opposite directions: a high value usually flags liver cell injury, while a very low value often flags a body that is losing muscle and reserve. In adults over 65, a higher ALT within the normal range has tracked with slightly lower death rates, because at that age it can partly reflect a well-preserved body rather than a damaged liver. The direction of the risk depends on why the number moved.
The single biggest misconception about ALT is that a normal value clears the liver. It does not. In one series of people with cirrhosis from alcohol, about 90% had normal ALT. In chronic hepatitis B, a meta-analysis found that around one in five patients with ALT in the normal range still had meaningful liver scarring.
Older lab upper limits are part of the problem. Large studies keep finding that healthier reference limits are lower and sex-specific, with women's limits below men's. A value that a lab flags as normal can still sit above the level where risk starts to rise, especially for women.
ALT can move around within the same person even when nothing meaningful has changed. In tightly controlled weekly sampling, the usual swing was around 9%; older studies reported wider swings. A single value near a cutoff is noisy by nature.
Three other factors can move a reading without meaning much:
A note on statins, which people often blame: a mild ALT bump on a statin is usually harmless and does not mean the drug is damaging your liver. In fatty liver, statins more often lower ALT than raise it.
A single ALT tells you where you were at one draw. A series tells you whether the liver-injury signal is drifting up, holding steady, or falling after the cause is treated.
A falling ALT can be useful feedback when the cause is clear, such as fatty liver improving with weight loss or hepatitis C responding to treatment. But ALT is not the whole liver. Fibrosis can worsen while ALT looks quiet, so pair the trend with the cause and the companion tests.
ALT should not be asked to answer a fibrosis question by itself. If it is high, pair it with AST, GGT, ALP, bilirubin, platelets, and a fibrosis score. If you have diabetes, prediabetes, heavy alcohol use, or other liver risk, consider imaging or elastography even when ALT looks ordinary.
The pattern matters. ALT and AST high together point one way; GGT and bilirubin point another; low platelets with an AST-heavy pattern raise concern for scarring. Persistent unexplained elevation or signs of scarring call for liver-focused care.
