About Tumor Necrosis Factor Alpha Genotype

If you already have rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, psoriasis, or inflammatory bowel disease, one practical question can matter a lot: will a TNF blocker help enough to justify months on it? Most people find out by trying the drug and watching. This test adds a DNA-based clue before that trial starts.

TNF stands for tumor necrosis factor alpha. It is one of the immune system's inflammation signals. This test reads inherited variants in the TNF gene from cheek cells collected with a swab. The result is a permanent genotype, not a measure of how inflamed you are today.

The evidence is strongest for drug response, but no TNF promoter variant is guideline-endorsed for choosing or rejecting a biologic. No guideline currently uses a TNF promoter genotype as a prescribing rule. The one anti-TNF pharmacogenetic marker that has entered guideline discussion is HLA-DQA1*05, a separate gene covered later in the article, not any TNF variant.

What This Test Actually Reads

TNF is a protein immune cells release to start and amplify inflammation. It helps drive fever, white blood cell recruitment, and tissue injury when inflammation stays switched on. That is why TNF-blocking drugs such as adalimumab, infliximab, and etanercept are standard treatments in several inflammatory diseases.

The TNF gene has small spelling variants in its promoter. The promoter is the gene's on switch. The most studied position is -308G/A, also called rs1800629. Other commonly reported promoter variants include -238G/A, -857C/T, and -1031T/C. Your report should be read variant by variant, because evidence for one position does not automatically apply to another.

Blood, saliva, and cheek-swab studies can all be relevant to a germline genotype because the inherited DNA call should be the same in each cell type. The mismatch to watch for is different: studies that measure TNF protein in blood or saliva are measuring a changing inflammatory signal, not this fixed genotype.

Predicting Anti-TNF Drug Response

Drug response is the most useful question for this test, though the evidence is still short of guideline-grade. A 2026 meta-analysis of ankylosing spondylitis and psoriatic arthritis combined 11 comparisons across 611 people (453 responders, 158 non-responders) and found that carriers of the -308 G allele were about four times as likely to respond to TNF blockers (OR 4.22, 95% CI 1.69 to 10.54). The confidence interval is wide, so the direction is more reliable than the exact size.

Rheumatoid arthritis data point the same way, but not cleanly. One 15-study meta-analysis found better response among people with the G allele. A European-ancestry-only meta-analysis found no association at -308 (OR 1.24, 95% CI 0.98 to 1.56), and an updated meta-analysis found a signal only in small studies of fewer than 100 patients. An individual UK cohort tied the rare AA genotype to poorer etanercept response only. This is why the result belongs beside diagnosis, disease activity, prior drug history, and anti-drug antibody testing, not above them.

In the 515-subject Chinese study used in many discussions of this test, 373 participants had ankylosing spondylitis, rheumatoid arthritis, or psoriatic arthritis and 147 received a TNF inhibitor. Non-response was rare, so the statistic is fragile: six of the ten non-responders carried GA at -308. People with GG were more likely to respond.

The inflammatory bowel disease evidence is newer and smaller. In an 83-person European cohort, GG at rs1800629 was more common among people who reached steroid-free remission: 97.7% versus 82.8%. A-allele carriers stopped infliximab or adalimumab more often, with an estimated risk about six and a half times higher. That estimate is imprecise because the cohort was small, and broader IBD pharmacogenomics reviews find TNF-region variants have shown inconsistent associations with anti-TNF response overall.

A GA or AA result does not rule out TNF blockers. Many carriers still respond. It shifts the odds, which may matter if you and your specialist are choosing between drug classes.

Disease Susceptibility and Ancestry

TNF variants also show ancestry-specific disease associations. These are small-effect findings. They shift odds in groups, but they do not diagnose disease in one person.

The two psoriasis rows show why a simple "G = good, A = bad" heuristic does not hold. Allele effects are disease- and direction-specific: the same -308 A allele that shifts anti-TNF response the wrong way in inflammatory arthritis appears to lower psoriasis risk. Read your report position by position.

These associations are not strong enough to change screening when you have no symptoms. They matter as background if your family history or diagnosis already points in the same direction.

Oral Health

Oral-health studies are relevant when they genotype TNF itself. The mouth is the disease site in those studies; the genotype would be the same from a cheek swab or blood.

In 378 Brazilians, the -308 A allele was linked to lower odds of persistent apical periodontitis, a long-lasting infection at the root of a tooth. In chronic gum disease, GA and AA at -308 were linked to higher TNF protein in inflamed gum tissue, even after accounting for the main gum-disease bacteria. Same trigger, stronger local inflammatory response.

Oral cancer data are mixed. Some cohorts link TNF promoter variants with oral squamous cell carcinoma, while a Taiwanese study found a different direction at -308 than many people would expect. This test is not an oral cancer screen. Tobacco, alcohol, HPV exposure, and visible mouth lesions matter far more.

Rheumatoid Arthritis and Lupus

Meta-analyses of -308 and rheumatoid arthritis do not show a broad risk signal across everyone. The signal appears in some ancestry groups, including Latin American cohorts, and disappears in others.

Lupus evidence is also ancestry-dependent, and meta-analyses have not all agreed on the same group. That makes this a weak disease-risk test. Its better use is after diagnosis, when a TNF-blocking drug is being considered.

Severe Illness and Sepsis

A different nearby variant, TNFB2 in the lymphotoxin-alpha gene region, was tied to higher circulating TNF and higher mortality in one severe-sepsis cohort. That is not the same as -308G/A in TNF.

Meta-analyses of -308G/A suggest possible links to sepsis or septic shock susceptibility, but not to sepsis mortality. Individual studies remain mixed. This result should not reassure or alarm you about how you would do in critical illness. Infection source, age, organ failure, and treatment timing matter more.

Reading Your Result

Most reports describe -308 as GG, GA, or AA. GG is the common genotype and the one more often linked to TNF-blocker response in several studies. GA carries one A allele; it is the genotype most often tied to weaker response. AA is uncommon, so many studies are too small to estimate its effect well.

The result is a genotype, not a level. It does not tell you today's TNF protein concentration or your current inflammation burden. For that, hs-CRP, ESR, a CBC, and disease-specific scores are more useful.

Why This Is a One-Time Test

Your germline DNA does not change. The variants you inherit at conception are the variants you carry for life. There is no trend to follow, no seasonal drift, and no lifestyle change that turns GG into GA.

What does need tracking is the biology showing up in your body now. If you have an inflammatory disease, that can mean hs-CRP, ESR, CBC-derived ratios, fecal calprotectin for IBD, disease activity scores, drug levels, or anti-drug antibodies. The genotype tells you a fixed tendency. The phenotype tells you what is happening.

What an Unexpected Result Should Trigger

If you get GA or AA and you already have an inflammatory disease, put it in the file used for biologic decisions. It does not veto a TNF blocker. It may make another drug class or closer follow-up more attractive, depending on your diagnosis, previous response, antibodies, drug levels, and safety history.

If you have no inflammatory disease, the action is small. A TNF variant is a susceptibility factor among many. Share it with biological relatives only if it is useful for a shared disease history or a drug decision they may face.

If a result would change a high-stakes decision, confirm it by a different method such as Sanger sequencing. This is about ruling out a technical call error, not repeating the same marker over time.

When Results Can Mislead

The biggest limit is panel coverage. A negative result only means the tested TNF positions were negative. It does not rule out rare TNF variants, TNF receptor variants, HLA-DQA105, or the many other genes that shape inflammation and anti-drug antibody risk. HLA-DQA105 is the one anti-TNF pharmacogenetic marker with guideline traction: carriage (present in about 40% of Europeans) roughly doubles the risk of forming anti-drug antibodies and is discussed in the 2025 ACG Crohn's disease guideline, while no TNF promoter variant has reached that status.

Ancestry also changes the read. Allele frequencies and risk estimates differ across populations. A number from a Chinese, Brazilian, Romanian, or European cohort may not transfer cleanly to you.

Variant labels matter. A known variant with published response data is not the same as a variant of uncertain significance from a broad sequencing panel. If a report calls a finding uncertain, treat it as uncertain.

Clinical-grade and direct-to-consumer genotyping are not interchangeable. A medical decision should rely on a lab method meant for clinical use, with identity checks and confirmatory testing when the result is surprising.

Somatic changes are a different problem. A cheek swab is meant to capture inherited DNA, but a history of bone marrow transplant, blood cancer, or major sample contamination can make germline testing harder to interpret. If that applies to you, confirm the result before acting on it.
