This test is most useful if any of these apply to you.
If you drink from a private well, eat rice most days, smoke, or live near old mining or industrial land, arsenic can get into your body for months with little to warn you. Hair can keep a record of that exposure, but it isn't a clean record. Some arsenic enters the shaft while hair is forming. Some can stick from the outside.
Hair arsenic is an exposure-investigation marker, not a routine screening test with universal cutoffs. It will not diagnose a disease, and it cannot tell you whether arsenic is harming you today. The useful question is narrower: does your hair show enough of an exposure signal to justify urine speciation, water testing, and a search for the source?
Arsenic (chemical symbol As) is naturally occurring, and your body does not make it. It gets in from outside through water, food, smoke, dust, or soil. Hair is built mostly from keratin. That protein contains sulfur sites that arsenic can bind to. Once arsenic is incorporated into the shaft, it can stay there as hair grows out.
That makes hair a record of the past, not the present. Blood is most useful for very recent or ongoing exposure. Urine reflects the last several days and is the better validated clinical test, especially when it separates inorganic arsenic and its breakdown products from seafood arsenic. Hair grows roughly a centimeter a month, so the piece nearest the scalp is the newest part of the record. Segment testing can estimate timing, but surface contamination can still confuse it.
The biggest source worldwide is contaminated groundwater, which is why private well users are the classic at-risk group. Food matters too. Rice and other crops grown in arsenic-rich soil or water can carry inorganic arsenic into the diet, and scalp hair arsenic has tracked with arsenic in rice and vegetables in exposed communities. Smoking adds more, because tobacco plants can take up arsenic from soil.
The form matters. Most harm from environmental arsenic comes from inorganic arsenic, especially from contaminated water and some foods. Most finfish and shellfish arsenic is a far less toxic organic form and leaves quickly in urine. Seaweed is less tidy, and some dietary forms can blur urine interpretation. A total hair test does not sort the form or source, so the result has to be read with your water, diet, work, and hair-product history.
The trap is treating total hair arsenic as a direct dose meter. It isn't. A high result can come from internal exposure, from contaminated dust or water sitting on the hair, or from lab and washing differences. It may also be affected by seafood or seaweed patterns, though seafood is a bigger problem for total urine than for hair. Use a speciated urine test to see whether inorganic-related arsenic is high, test drinking water, and look for a source you can remove.
The most established human sign of chronic arsenic exposure is in the skin: thickened patches on the palms and soles, and darkening or lightening of pigment. In South and Southeast Asian cohorts, including West Bengal and Malaysia, higher hair arsenic was associated with arsenic-related skin lesions. Chinese studies of arsenic-endemic areas show the same skin pattern with drinking-water exposure. These changes often appear years into exposure, which is a reason to find the source earlier.
Chronic exposure to inorganic arsenic is a proven human carcinogen, tied most strongly to cancers of the skin, lung, and bladder. Evidence for kidney, liver, and prostate cancer is more limited. The harm builds over years. Arsenic can create unstable oxygen chemicals inside cells and interfere with DNA repair. A hair test does not detect cancer or predict it for one person. It can flag a possible exposure source while there is still time to cut it off.
Arsenic exposure also shows up in the cardiovascular and metabolic system. Among 398 non-working women in one Chinese study, higher scalp hair arsenic came with about two and a half times the odds of high blood pressure, a stronger figure than the more modest effect seen when studies are pooled together. In Bangladeshi studies, higher water, hair, and nail arsenic tracked with more damaged LDL particles, lower HDL, higher CRP, and more insulin resistance. CRP is a blood marker of inflammation. These are population patterns, not a verdict on any single reading.
Higher hair arsenic has been linked to worse cognition in exposed adults. In a 1,556-person study near a realgar plant in China, cognitive impairment was more common in people with higher hair arsenic, and arsenic-related illness carried about 1.8 times the odds of impairment. This is a group-level association. It cannot tell you what a given hair result means for your memory on its own.
Start with the uncomfortable fact: commercial hair mineral testing has a record of poor agreement. When investigators split one person's hair and sent it to six US labs, results for some minerals varied more than tenfold, and the same sample was called high by one lab and normal or low by another. That alone means a single hair number, read in isolation, is weak evidence.
Then there is contamination the lab cannot fully see. Hair sits in the open and binds arsenic from dust, water, sweat, shampoos, dyes, and other hair treatments. Washing helps, but it does not fully solve the problem. Bleaching, perming, and dyeing can remove arsenic or add it, distorting the result either way.
Hair arsenic works best when the source question is concrete: a private well, a worksite, a nearby smelter, rice from a high-arsenic region, or known arsenic in dust or soil. Vague symptom hunting is where hair mineral testing goes wrong.
