This test is most useful if any of these apply to you.
Beta-2-microglobulin has an established role in assessing prognosis in multiple myeloma and some other blood cancers. Its concentration reflects both protein production and kidney clearance, so a high result can come from cancer activity, inflammation, infection, reduced kidney function, or a combination.
Research also links higher concentrations with future kidney failure, cardiovascular disease, and death. Those associations haven't established that screening healthy people with this test improves outcomes. Its clinical value depends on the question you're trying to answer.
Most cells use proteins on their surface to show the immune system fragments of what's inside them. B2M forms part of this structure, called the MHC class I complex, and is released into the circulation. Increased production during immune activation or certain blood cancers can raise its concentration.
B2M passes through the kidneys' filters. The kidney tubules then take up and break down most of the filtered protein. Reduced filtering allows it to accumulate in the blood, even without increased production.
This test reports serum B2M in mg/L. Serum is the liquid remaining after blood clots. Plasma is separated from blood before clotting. Several studies discussed below measured plasma, so they provide related evidence rather than direct validation of this serum measurement.
Multiple myeloma is a cancer of the antibody-making plasma cells in bone marrow. Its International Staging System combines serum B2M with albumin. The revised system adds lactate dehydrogenase and high-risk genetic changes in the cancer cells, and its second revision, increasingly used, also counts an extra copy of part of chromosome 1. B2M contributes because it reflects both disease activity and kidney function; it isn't a direct measurement of tumor size.
The 2025 International Myeloma Society and International Myeloma Working Group definition of high-risk myeloma counts a high B2M only when creatinine is normal. That's the clearest guideline example of reading this number against kidney function. NCCN now lists this definition alongside the three staging systems.
Early studies established its prognostic value. In a small study published in 1980, median survival was 46 months in the lower-B2M group and 15 months in the higher group. Those figures describe people treated decades ago and shouldn't be used to estimate your prognosis with current treatment.
In a study of 148 people with asymptomatic myeloma, the group with higher serum B2M had about three times the rate of progression to active disease after adjustment for other measured risk factors. B2M also contributed to a model predicting progression in asymptomatic Waldenström macroglobulinemia. The current international risk model for smoldering myeloma tested B2M as a candidate factor, but the criteria in clinical use rely on bone marrow plasma cells, the abnormal antibody protein, and the free light chain ratio instead. These findings concern people with an existing diagnosis, and B2M alone doesn't determine whether treatment should start.
The prognostic association extends to other blood cancers. Among 915 people with Hodgkin lymphoma, 70% in the higher-B2M group remained free of progression at 10 years, compared with 83% in the lower group. The dividing line the study selected fell inside the normal reference interval, so a result within normal limits still carried prognostic information. In diffuse large B-cell lymphoma, adding B2M slightly improved a standard prognostic model in an analysis of 3,232 people with complete data.
A study of early chronic lymphocytic leukemia found that B2M remained associated with survival in people with impaired kidney function. That study measured plasma B2M. Kidney impairment still complicates interpretation of an individual serum result.
A normal result can occur despite substantial disease. In a small imaging study of 52 people with myeloma, which excluded people with kidney failure, 24 of the 35 with five or more bone lesions had B2M within their laboratory's reference interval. Blood proteins, bone marrow findings, and imaging answer different parts of the diagnostic question.
The Atherosclerosis Risk in Communities study measured serum B2M in adults without cancer. Compared with the lowest quarter, the highest quarter had about 25% more subsequent cancers and roughly twice the colorectal cancer risk after adjustment for age, sex, race, smoking, body mass index, and other measured factors. The associations persisted after further adjustment for inflammation. The published summary doesn't list kidney function among the adjustments, so reduced clearance may account for part of the link.
Genetic analyses have also explored whether B2M might contribute to cancer development. These studies use inherited variants to examine possible causal relationships. One analysis linked genetically predicted circulating B2M with diffuse large B-cell lymphoma and Hodgkin lymphoma, and a Finnish dataset also suggested a link with follicular lymphoma. Its underlying protein studies included serum and plasma measurements. Several of the variants sit in genes that govern kidney function, so they may act through kidney clearance rather than B2M itself. The result doesn't establish that lowering serum B2M prevents these cancers.
These findings haven't validated B2M as a stand-alone cancer screening test. Its established cancer uses concern prognosis and selected monitoring after diagnosis. B2M is also one component of the original five-protein OVA1 test, which helps assess an already identified ovarian mass when surgery is planned. That use doesn't extend to screening people without a mass.
Serum B2M generally rises as kidney filtering declines. It has been studied as an additional filtering marker and as a predictor of kidney disease progression, but inflammation and increased protein production can weaken the relationship between its concentration and actual kidney function.
An often-cited early study found that B2M detected reduced filtering when creatinine remained within its reference interval. It measured plasma B2M in just 12 people with insulin-dependent diabetes and protein in their urine. That finding doesn't establish that serum B2M routinely detects kidney disease before modern kidney assessments.
There is direct serum evidence in other settings. A retrospective study in hospitalized children found that serum B2M distinguished acute kidney injury reasonably well and rose with injury severity. Among 408 people with biopsy-confirmed diabetic kidney disease, higher serum B2M predicted subsequent dialysis or transplantation and added information beyond creatinine. These were selected clinical populations.
The large Chronic Renal Insufficiency Cohort study also linked B2M with kidney failure, cardiovascular disease, and death after accounting for kidney function. Although its original publication described serum measurements, a later correction states that the samples were plasma.
A pooled analysis of individual data from about 23,000 people found that B2M-based filtering estimates modestly improved prediction of kidney failure and death over creatinine-based eGFR. Once urine albumin was included, they added little further information.
In type 2 diabetes, a research model combining serum B2M with kidney injury molecule 1 performed similarly to much larger biomarker panels for predicting rapid kidney decline. Kidney injury molecule 1 is a protein associated with damage to kidney tubules. This finding supports further evaluation of the combination; it doesn't establish a routine testing strategy.
The standard assessment remains an estimated glomerular filtration rate, or eGFR, together with urine albumin. eGFR estimates how well your kidneys filter. Combining creatinine and cystatin C usually gives a more accurate estimate than creatinine alone. Agreement between these markers and B2M supports reduced clearance as a contributor, but it can't exclude simultaneous inflammation or cancer activity.
B2M accumulates when kidney function is severely reduced. In the HEMO study's analysis of 1,704 people receiving hemodialysis, higher predialysis serum concentrations were associated with higher mortality after adjustment for factors including residual kidney function. The dialyzer's B2M clearance wasn't independently associated with mortality.
An international dialysis study in Europe and Japan analyzed B2M and mortality in 5,332 people. Those in the highest third had about 38% higher mortality than those in the lowest third. A separate, larger analysis in the same publication examined hospitalizations related to dialysis-associated amyloidosis. In this condition, B2M forms deposits that can damage joints and nearby tissues. Those hospitalizations fell more than tenfold between 1998 and 2018.
The relationship isn't uniform across dialysis populations. Among 725 people starting peritoneal dialysis, lower B2M was associated with worse survival. A Japanese national peritoneal dialysis registry found the opposite: higher B2M was independently linked with higher mortality, whether people stayed on peritoneal dialysis or moved to combined therapy or hemodialysis. Residual kidney function, dialysis removal, inflammation, and nutritional status can all affect these relationships. The studies don't establish that low B2M itself causes harm or that protein production is the main source of variation in everyone receiving dialysis.
Higher circulating B2M is consistently associated with cardiovascular disease and mortality in observational studies. The size of the association depends on the population and how well the analysis accounts for kidney function, age, and other illnesses.
| Who Was Studied | What Was Compared | What the Study Found |
|---|---|---|
| About 31,000 people across 16 studies, mostly involving kidney disease | Highest versus lowest third of circulating B2M; studies included serum and plasma | About 71% more cardiovascular disease. Cardiovascular mortality was about 2.3 times as high, and all-cause mortality about 2.5 times as high. |
| 10,388 US adults with about 18 years of follow-up | Highest versus lowest quarter of serum B2M | About 2.5 times the all-cause mortality and 2.6 times the cardiovascular mortality after adjustment, with similar findings in people without chronic kidney disease. |
| Postmenopausal women in a US survey | Serum B2M and other markers in relation to existing cardiovascular disease | Higher B2M was associated with existing disease. This cross-sectional comparison didn't establish prediction of future events. |
The pooled results varied substantially between studies. Statistical adjustment also can't fully separate B2M from the illnesses that raise it. In a study of 1,302 older adults, age, male sex, and inflammation were associated with serum B2M beyond their relationship with estimated kidney function.
These findings make B2M a potentially useful research marker of risk. They haven't shown that adding it to routine cardiovascular assessment changes treatment decisions or improves survival, or that lowering the number itself prevents heart disease.
Immune activation can raise serum B2M. In a small inflammatory bowel disease study, average concentrations were about 40% to 45% higher during active ulcerative colitis or Crohn's disease than during remission. Autoimmune disorders and viral infections can also raise it, so an increase doesn't identify a particular cause.
Studies in hospitalized people have linked higher serum B2M with more severe COVID-19 and with short-term mortality in bacterial infection causing severe lung injury. A retrospective pediatric study found higher concentrations in viral than bacterial upper respiratory infections. These findings haven't established B2M as a stand-alone way to identify an infection or decide whether antibiotics are needed.
In 158 older adults, higher serum B2M was associated with weaker grip, slower walking, and poorer physical performance. Related experiments found that administering B2M caused muscle wasting in mice. The human study showed an association, and the mouse experiment doesn't establish that B2M causes muscle loss in people.
Another study of 846 cognitively healthy adults linked higher plasma B2M with poorer cognitive performance and Alzheimer's-related findings in spinal fluid. This was a cross-sectional study using a different blood specimen. It doesn't validate serum B2M for diagnosing Alzheimer's disease or measuring biological age.
Repeat-measurement studies found relatively low short-term variation in serum B2M, similar to creatinine and cystatin C. That helps when comparing measurements, but it doesn't provide a universal definition of a meaningful change or prove that annual testing detects disease early.
A trend is most useful when tied to an established condition and interpreted with its other measurements. In myeloma, formal response assessment relies on tests such as the abnormal antibody protein, free light chains, bone marrow findings, and imaging when appropriate. B2M can add context, but a change alone doesn't establish remission or relapse.
