About FFRCT - LM

The left main coronary artery supplies most of the main pumping chamber of your heart. A serious narrowing there is one of the few findings in cardiology that can change what happens next, and fast.

A regular heart CT scan can show that this artery looks narrowed. This score goes one step further and estimates whether the narrowing is actually holding back blood to the muscle. That difference, between looks narrow and is starving the heart, is useful across the coronary arteries. For a significant left main narrowing, though, the decision leans toward the cath lab regardless, so the flow score adds more to reading the disease around the left main than to the left main call itself.

What the Score Is Measuring

FFRCT (fractional flow reserve derived from CT) is not a substance in your blood. A coronary CT angiogram is a scan of the arteries feeding your heart. FFRCT is a number a computer calculates from those pictures.

The number here is the calculation at the left main artery. It runs from 0 to 1 and estimates how much pressure is lost as blood pushes past a narrowing. A value near 1 means blood flows through freely. A lower value means the narrowing is holding blood back. Values close to the usual clinical cutoff are less reliable than values that are clearly reassuring or clearly concerning, so borderline results often need an invasive check before anyone acts.

Why the Left Main Gets Treated Differently

The left main is short, but it splits into the two arteries that supply the front and side of the heart. Between them they cover most of the muscle. Because so much depends on this one segment, cardiologists treat less narrowing in the left main as serious than they would in a smaller branch. A flow-limiting left main lesion is dangerous in a way a similar lesion in a smaller branch is not. That is also why a significant left main narrowing goes to invasive angiography regardless of what a CT flow number says.

What a Low Score Predicts

Across studies of FFRCT in all coronary arteries, not the left main alone, a low value marks people at higher risk. In the current expert-consensus summary, a positive result predicts roughly two to three times the risk of heart attack and cardiovascular death. Some pooled analyses report a larger relative risk for composite major cardiac events, but those composites were driven partly by later artery procedures rather than hard outcomes like death or heart attack.

A low value also predicts what happens next. In the ADVANCE registry, about 38 of every 100 people with a low result had an artery opened within a year, versus about 6 of every 100 with a reassuring result. That was across coronary arteries, not left main disease alone.

For you, a low left main value is not a number to sit on. It is a reason to get in front of a cardiologist quickly, because the left main is where the stakes are highest.

How It Compares to a Plain CT Scan

A standard coronary CT angiogram shows anatomy: where the plaque is and how narrow the channel looks. Its weakness is false alarms. An artery can look tight and still carry enough blood, or look moderate and be flow-limiting. Adding the flow calculation mainly helps sort those apart.

Against the invasive pressure wire used in the cath lab, FFRCT catches about 89 of 100 people who truly have a flow-limiting narrowing and correctly clears about 71 of 100 who do not, in one widely cited pooled analysis; other meta-analyses put accuracy somewhat lower. Its edge over the plain scan is specificity, fewer false alarms. In one randomized trial, a CT-first strategy with selective flow analysis cut invasive angiograms from about 1 in 4 people down to about 1 in 5. The 2021 AHA/ACC chest pain guideline supports the flow calculation for 40 to 90 percent narrowings in stable chest pain, but not for the left main.

Where the Left Main Evidence Runs Thin

Left-main-only validation is thin, and expert consensus does not endorse the CT flow score as a gatekeeper for the left main. Most studies test FFRCT across all major coronary arteries, then apply the same physiology to the left main. In the main heart-team study of left main and three-vessel disease, doctors reading the CT scan reached nearly the same treatment decisions as doctors reading the invasive angiogram. Adding the flow calculation changed the plan in only about 1 in 14 patients.

A significant left main stenosis, roughly a 50 percent or greater narrowing, warrants invasive angiography with intravascular imaging or an invasive pressure wire regardless of the CT flow value. The validated role for FFRCT is different: it gates intermediate narrowings outside the left main and helps plan treatment in multivessel disease. In the left main itself, the anatomy is often severe enough that the picture alone decides the plan. Two large trials found that adding flow analysis reduced unnecessary catheter procedures without clearly improving survival, and one increased the number of arteries opened without easing symptoms. Read it as an adjunct that sharpens planning and spares procedures, not a verdict on the left main.

When the Score Can Mislead

The biggest problem for a left main reading is calcium. Heavy calcium buildup shows up bright on CT. It can blur the channel and make the artery look tighter than it is. In studies of people with very high calcium scores, FFRCT often stayed useful but lost some of its edge over the plain scan, and the evidence is mixed. No single calcium score rules the test out; what matters is whether the images are clean enough to trust.

  • Heavy calcium: bright calcified plaque distorts the artery outline and can drag accuracy down, though results across calcium levels are mixed.
  • Image quality: a fast or irregular heartbeat, or motion during the scan, can smear the pictures the calculation depends on.
  • Borderline values: results near the usual clinical cutoff are the least reliable and often need an invasive check before anyone acts.
  • Vessels it cannot cover: the calculation is not computed in fully blocked arteries, very small or distal segments, stented segments, or bypass grafts.
  • Downstream disease: narrowings in the left anterior descending or circumflex arteries can complicate how left main physiology is read.

Why One Scan Is a Snapshot, Not a Trend

Unlike a blood test you can repeat every few months, this comes from a CT scan that uses radiation and contrast dye. You do not run it on a schedule. It is a point-in-time read on whether your left main is flow-limiting today.

What you track over the years is the disease underneath it. If your score is reassuring, the job is to keep it that way by driving down what builds the plaque. If it is borderline or low, the follow-up lives partly in the cath lab and partly in your blood work. ApoB tracks the plaque-building cholesterol particles. Symptoms matter too. A repeat scan is warranted only if your situation changes, not as routine monitoring.

What to Do With an Abnormal Result

A low left main value should trigger a specific chain of events, not a wait-and-see. First, take it to a cardiologist, ideally one who can convene a heart team. A heart team brings together cardiology and cardiac surgery to decide between a stent, bypass surgery, or medical therapy. Left main disease is one of the cases where that conversation matters and should not be delayed.

Expect an invasive angiogram, often with intravascular imaging or a pressure wire, to confirm the finding, especially if your calcium score is high or your value is borderline. In the left main the invasive pressure measurement itself takes extra care, so it is done by an experienced team. Alongside that, the drivers of the plaque get measured and treated hard: ApoB, lipoprotein(a), and inflammation. The scan tells you the artery is threatened. The rest of the workup tells you why, and what to do about it for the long run.
