About Linear PFOS (n-PFOS)

Almost everyone walking around today carries measurable amounts of this chemical in their blood. It is one of the so-called forever chemicals, built to resist breakdown, and once it enters your body it can linger for years.

That persistence is why the number matters. Higher levels have been tied to higher cholesterol, higher blood pressure, and early signs of liver strain, so knowing your level tells you how much of this exposure your body is quietly holding onto.

What You Are Actually Measuring

In blood, linear PFOS (perfluorooctane sulfonic acid) is a synthetic industrial chemical, not something your body makes. It belongs to a large family of man-made compounds called PFAS (per- and polyfluoroalkyl substances), used for decades in nonstick coatings, stain repellents, food packaging, and firefighting foam.

The word linear refers to the straight-chain form of PFOS, one of two shapes the molecule comes in. The other is the branched form, and labs can measure the two separately. Your body has no natural source of either and cannot break them down, so a blood level reflects how much you have absorbed from the outside world.

Cholesterol and Lipids

The most consistent finding is that higher PFOS goes hand in hand with a worse cholesterol picture. In two large general-population studies, higher PFOS was tied to higher total cholesterol and higher fat content across nearly every lipoprotein fraction, even at the low exposure levels typical of the general public.

The pattern holds in more exposed groups too. In communities with contaminated drinking water, each step up in PFOS exposure was associated with roughly 5.7 mg/dL higher total cholesterol and about 4.9 mg/dL higher non-HDL cholesterol. A pooled analysis of adults similarly linked higher PFOS to higher total cholesterol and higher LDL, the fraction most tied to heart disease.

What this means for you: if your PFOS is elevated, it is worth pairing the result with a full lipid panel. A rising cholesterol number in someone with high exposure is a signal to look at both diet and exposure, not just one.

Blood Pressure

Evidence on blood pressure is real but not uniform. In a long-running study of midlife women who started free of high blood pressure, those in the highest third of PFOS were about 42% more likely to develop hypertension over follow-up than those in the lowest third.

A separate cohort of adults with prediabetes found mostly no link between baseline PFOS and later hypertension. The 42% figure comes from a study of midlife women specifically, while a broader meta-analysis found the link somewhat stronger in men. So the association appears in some populations but is not uniform, and it should not be read as a guaranteed effect. It is one more reason to track blood pressure closely if your exposure is high.

Liver Strain

PFOS accumulates heavily in the liver, and higher blood levels track with early markers of liver-cell strain. In a very large study, higher PFOS was associated with higher ALT, the most direct blood signal of liver-cell injury, and one 2025 study linked serum PFOS to higher odds of fatty liver disease.

The size of these effects is usually small. Another national analysis found that PFOS effects on liver enzymes were modest and weaker than the effects of alcohol or smoking. The signal is worth watching, but a single elevated PFOS does not mean your liver is damaged.

Metabolic Signals

PFOS has been connected to broader metabolic disturbance. In midlife women, higher PFOS was associated with about 7.8% higher free leptin index, a marker tied to appetite regulation and body fat, and a PFAS mixture that included PFOS was linked to higher risk of developing diabetes. Detailed metabolic profiling of thousands of adults found PFOS related to a wide range of fat and sugar-handling molecules, with signals that persisted even after accounting for a related PFAS.

Cancer Signals, Still Emerging

At the very high end of exposure, one small nested study found that people with PFOS above about 55 micrograms per liter (a high concentration seen mainly in heavily exposed people) had roughly 4.5 times the risk of non-viral liver cancer. This finding is preliminary. The largest study to date, pooling 853 liver cancer cases across 12 cohorts, found no overall link between blood PFOS and liver cancer, so the earlier signal has not held up in larger groups.

For breast cancer, the picture is mixed and often specific to tumor type. PFOS was not clearly linked to breast cancer overall in postmenopausal women, but was associated with higher risk of hormone-receptor-positive tumors in subtype analyses. A separate study that separated the isomers reported increased risk for a branched form of PFOS rather than the linear form measured here, which underlines that these two shapes can behave differently.

Pregnancy and Early Life

PFOS crosses the placenta, so a mother's exposure becomes the baby's exposure. Interestingly, the linear form transfers across the placenta less efficiently than the branched form, but transfer still happens, and PFOS appears in both maternal and cord blood.

A pooled analysis linked higher PFOS around pregnancy to a birth weight deficit of about 30 grams, and placental studies suggest the effect may differ by the baby's sex. The evidence is not specific to the linear form alone, but it is a reason many people want to know their level before or during pregnancy.

The Kidney Puzzle

One finding looks backward at first glance: in a cohort of young adults, higher PFOS came with higher kidney filtration rates, which sounds like a good thing. It is not. Two explanations resolve the paradox. In young people, a temporarily high filtration rate can be an early sign of kidney stress rather than kidney health. And in single-snapshot studies, people whose kidneys work well clear PFOS faster, so a lower level can be a result of good kidney function rather than the cause of it. PFOS is best read as an exposure marker, not a number where a higher filtration rate means you are doing better.

What Your Number Reflects Over Time

A single PFOS reading captures years of accumulated exposure, not a recent meal or a bad week. That is because the body clears it slowly. Estimates for total PFOS cluster around 3.4 to 5.7 years to fall by half, and isomer-specific work put the linear form at about 2.73 years. Either way, the number moves on the scale of years.

This is exactly why a trend beats a single value. Population averages have been dropping since regulation, falling by roughly 14.8% per year in one study of midlife women as older exposures worked their way out. Your own trajectory tells you whether your exposure is ongoing or fading.

A practical cadence: get a baseline, and if you are actively reducing exposure or donating blood, retest in 6 to 12 months to see whether the level is coming down. Otherwise, rechecking every year or two is enough. Because the biomarker moves slowly, do not over-read small differences between two nearby readings.

When Results Can Be Misleading

  • Kidney function: because PFOS leaves the body mainly through the kidneys, people who clear it well tend to show lower levels, and impaired kidneys can shift the number in ways that reflect filtration rather than exposure.
  • Sex and smoking: men and smokers tend to carry higher PFOS on average, which matters when comparing your number to a general reference.
  • Pregnancy and breastfeeding: both act as exit routes that lower a mother's PFOS while transferring it to the baby, so a lower postpartum reading is not the whole story.
  • Recent versus older exposure: a single sample cannot separate direct PFOS you absorbed from precursor chemicals your body slowly converts into PFOS.

If Your Level Comes Back High

Start by confirming the pattern rather than acting on one draw. If the result is genuinely high, trace the likely source: contaminated drinking water, a job with foam or industrial exposure, or heavy seafood intake. Testing your water is often the highest-yield next step.

From there, pair the result with companion labs that show whether the exposure is having downstream effects: a full lipid panel, ALT and GGT for the liver, and kidney filtration. A broader PFAS panel is worth considering, because PFOS almost never travels alone, and seeing the whole mixture gives better context. If you are pregnant, planning a pregnancy, or occupationally exposed, a clinician or toxicology specialist should help you interpret the combination of findings.
