This test is most useful if any of these apply to you.
You cannot taste, smell, or feel per- and polyfluoroalkyl substances (PFAS), the synthetic forever chemicals used in nonstick coatings, water-repellent fabrics, and firefighting foam. Once they enter your body, they linger for years, and a routine physical will never look for them.
This panel measures nine of these chemicals from a single blood draw. The point is not to diagnose a disease. It is to turn an invisible exposure into a number you can compare against the general population and act on.
The central story is cumulative internal dose. These compounds bind tightly to proteins in your blood and clear slowly, so one draw reflects years of accumulated exposure rather than last week's water or last night's dinner. That is why a panel, not a spot check, is the right tool: it reads a burden that has been building quietly over time.
The combination also produces a source fingerprint that no single result can give. In contaminated communities, three of these chemicals (the ones known as PFOS, PFOA, and PFHxS) tend to rise and fall together and point toward polluted drinking water or firefighting foam. The longer-chain members of the group lean instead toward diet, especially seafood and eggs, and toward household products and dust. Reading them side by side tells you where your exposure is most likely coming from.
Two finer details round out the picture. The panel separates the straight (linear) and bent (branched) forms of PFOA and PFOS, and that ratio hints at whether the source is an older manufacturing process (electrochemical fluorination) or a newer one. It also includes MeFOSAA, a breakdown product rather than a final chemical, which captures exposure to precursor compounds that a panel of end-products alone would miss.
The tests were chosen to match clinical guidance. The National Academies of Sciences, Engineering, and Medicine (NASEM) recommends summing a set of these compounds into one serum value, then using that total to decide on follow-up. NASEM's guidance is built around seven compounds; this panel reports PFOS and PFOA as their separate straight and bent forms, which is why nine numbers appear, and those isomers are added back into the same total. Your results map onto the thresholds below, measured in nanograms per milliliter (a unit of concentration in blood).

The pattern within the total matters too. A result dominated by PFOS, PFOA, and PFHxS points toward a shared water or firefighting-foam source, and these three tend to move together closely. A result where the longer-chain compounds and the diet-linked members stand out suggests food and consumer products instead. A meaningful share of branched forms suggests an older manufacturing origin worth tracing.
Start with the source. Filtering contaminated water, checking your utility's testing reports, and reducing use of stain- and grease-resistant products are the actions that actually change your numbers over time. If your summed level is above 20 ng/mL, guidance suggests adding a thyroid stimulating hormone (TSH) test for adults, and, at older ages, checking kidney function and screening for kidney and testicular cancer and ulcerative colitis. These NASEM thresholds are the most widely used clinical framework, though not everyone agrees on them; some clinicians note there are no formally validated cutoffs tied to testing and no approved treatment, so the numbers guide monitoring rather than dictate care.
Pair the panel with the downstream markers these chemicals are linked to. Higher levels are most consistently associated with higher cholesterol in human studies, so total and LDL cholesterol are natural companions. In one study of teenagers, those in the highest exposure group were more than twice as likely to have abnormal cholesterol (odds ratio 2.24 for the summed chemicals). Kidney and testicular cancer show the strongest cancer signal in the research, though it remains observational.
Retest on a slow clock. Because these compounds leave the body over years, levels barely move in a few months. In one well-studied community, after exposure stopped blood levels fell by roughly a quarter each year for PFOA (about 26% per year), a fifth for PFOS (about 20% per year), and more slowly still for PFHxS (about 13% per year); other studies report somewhat different rates, but all agree the decline takes years. Retesting every one to two years shows whether the changes you made are working.
A few things shift the whole panel at once. Serum reflects historical exposure, so a level can stay high long after you fix a water problem, and current tap water often does not match current blood levels. Pregnancy and breastfeeding lower a person's levels because the chemicals are passed on, which can make serial results look like improvement when the source has not changed.
The panel is also deliberately focused on persistent, long-lasting compounds. Newer replacement chemicals (such as one called GenX) clear from blood much faster and will not show up here, so a clean result does not rule out every fluorinated exposure. Read the numbers as a strong estimate of your long-term burden, not a complete inventory of every chemical you have met.
