About Microalbumin, 24-Hour Urine with Creatinine

Kidney damage often shows up first as a leak. Albumin is a blood protein. In healthy kidneys, very little reaches the urine. When it is persistently high, the filtering barrier has usually been irritated or damaged, even while you feel fine and blood kidney tests still look normal.

This panel measures that leak across a full 24 hours and measures urine creatinine in the same jug. Albumin tells you how much protein was lost. Creatinine gives a rough sense of whether the full-day collection looks complete. The pair is the point.

What This Panel Reveals

Albumin in urine is a kidney signal, but not only a kidney signal. In diabetes and high blood pressure, persistent albumin leak is often the first measurable sign of kidney damage. It can appear before the estimated filtering rate falls, and long before swelling or other late symptoms.

The same leak points to blood vessel stress across the body. Human cohort studies link higher urine albumin with higher risk of heart attack, stroke, heart failure, and cardiovascular death, even after accounting for traditional risk factors. Risk doesn't start neatly at the official cutoff. It rises across the range.

Creatinine is the quality check. Your muscles make it at a fairly steady rate, and most of it leaves in urine. In a 24-hour collection, very low creatinine for your body size usually means some urine was missed, which can make albumin look falsely low. A normal creatinine makes the collection more believable but does not prove it was complete. That gap is one reason guidelines use the simpler spot urine ratio for routine screening and reserve the full-day collection for when the spot result is unclear or creatinine output is unusual.

How to Read Your Results Together

Albumin excretion has three standard bands. Under 30 mg in 24 hours is normal to mildly increased. From 30 to 300 mg is moderately increased, the older term was microalbuminuria. Over 300 mg is severely increased. Creatinine tells you how much trust to put in the collection.

The cutoff is useful, but it isn't magic. Large human studies show higher risk even below 30 mg/g on the spot urine ratio. That ratio is not the same as this 24-hour test, but it points to the same biology: less albumin in urine is generally better, especially if you already have diabetes, high blood pressure, or heart disease.

What to Do with Your Results

Don't diagnose yourself from one high value. Albumin varies enough that a borderline result can move across a cutoff. Diabetes guidelines confirm elevated albumin only when two of three samples over 3 to 6 months are high, after excluding short-term triggers. Pair this panel with a blood kidney test so albumin leak and filtering rate can be read together.

If the leak is confirmed, the result is actionable. Blood pressure, blood sugar, and kidney-protective drugs can all move albumin down in the right setting. Most outcome studies track the urine albumin-to-creatinine ratio rather than this exact 24-hour panel, so don't treat one percentage change as a law. Direction still matters: in chronic kidney disease with type 2 diabetes, a drop of more than 30 percent was tied to lower risk of death, heart events, and kidney decline, while a rise of more than 30 percent was tied to higher risk. Retest at least yearly if you have diabetes or known kidney disease, and one to four times a year once kidney disease is established.

When Results Can Be Misleading

Several things can push albumin up for a short time without proving chronic kidney disease: hard exercise in the previous day, fever, a urinary tract infection, menstruation, heart failure, marked high blood sugar, and marked high blood pressure. In established diabetes, though, high blood sugar can reflect real, ongoing kidney injury rather than a passing blip. A clean repeat after those triggers pass is often the deciding test.

Body size can bend the creatinine side. Heavier or very muscular people tend to put out more urine creatinine, which can make a spot albumin-to-creatinine ratio look lower than the true daily albumin loss. A full 24-hour collection helps catch that mismatch, as long as the collection is complete.
