Bismuth is the metal in Pepto-Bismol's active ingredient. Use it often enough and a small amount can get into your body, while some bismuth can also land on the outside of hair from products or dust. This test reads bismuth in hair, so it can hint at exposure over the last few months, but it can't prove where the metal came from.
That reading is exploratory, not diagnostic. There are no agreed hair-bismuth cutoffs, different hair labs can disagree, and for most people the number likely reflects low-level background exposure. It can't tell you by itself whether bismuth is hurting you.
Scalp hair grows about a centimeter a month and can hold trace metals as it forms, so a strand is closer to a slow record than a snapshot. In principle, hair bismuth (chemical symbol Bi) can point toward exposure over recent months rather than today. In practice, the test is weak for individual decisions because the amounts are tiny and outside contamination can look like true internal exposure.
The bigger problem is that hair mineral analysis is noisy. One study that split the same hair sample across commercial labs found conflicting results and reference ranges. Wash, dye, bleach, or heat-treat the hair and you can shift the reading. And a metal sitting on the outside of the strand from shampoo, dust, or sweat can look identical to metal that came from inside your body.
The most obvious source is the medicine cabinet. Bismuth subsalicylate, the active ingredient in Pepto-Bismol, is the everyday source. Bismuth is also used in four-drug H. pylori therapy. H. pylori is a stomach bacterium that can cause ulcers. Bismuth compounds also show up in some cosmetics, hair dyes, and dental materials.
With standard oral bismuth medicines, very little crosses into your blood, which is why short courses have a long safety record. In a 60-woman hair study, dyed hair and untreated hair had almost the same bismuth, about a 2 percent difference. Dyeing alone did not look like a strong source of bismuth specifically in that group, though the same processing can shift other metals more. So a mild hair reading rarely points to one obvious culprit.
One study is the reason bismuth shows up in conversations about children's health. In a comparison of 107 boys, those with ADHD had hair bismuth roughly eight times higher than boys without it, one of three individual elements that differed.
Don't read much into that. That specific bismuth signal was new, it hasn't become a replicated clinical finding, and the authors called the findings cautionary. One possible clue was diet, because canned fruit was eaten more often in the ADHD group. That doesn't make bismuth a cause. It remains a research lead, not a reason to test a child.
Bismuth can poison you, but usually after unusually high exposure. The clearest human evidence comes from outbreaks in France and Australia decades ago, where people took bismuth subnitrate or subgallate in gram-level daily doses for weeks to years and developed a brain syndrome: confusion, unsteadiness, trouble speaking, and muscle jerks. Those cases were tracked with blood and urine bismuth, not hair.
The kidney is the other target. It filters bismuth and can retain it after high exposure. A systematic review found kidney injury mostly after medical overdoses, with worse outcomes in people whose kidneys were already impaired. That's why kidney function matters more here than the hair number by itself.
The reassuring counterpoint is scale. When researchers measured blood bismuth in patients taking a standard H. pylori course, about one in a hundred had a blood level high enough to trigger extra monitoring, and none had a serious brain or nerve event. Toxicity and common use can both be true. Dose and kidney function do most of the work.
Start with the biggest weakness: hair metal results are not standardized. The same hair can give different numbers at different labs, and there's no agreed normal range to compare against. A handful of specific things can push a single reading in the wrong direction:
Because a single hair reading is noisy and has no personal baseline, treat your first result as context, not a verdict. If you reduce a clear source, the direction over time matters more than the exact figure. Hair doesn't cleanse itself; the sampled strand changes as new hair grows and older exposed hair is cut away.
A high hair number is most useful when it lines up with a known source or symptoms. A high hair result with normal blood or urine bismuth and no symptoms means something different from a high internal level in someone with reduced kidney function, confusion, tremor, or unsteady walking.
